To contact your DMG medical provider, please call the location at which the provider works. You can find this information on our website by searching for your DMG medical provider by last name, medical specialty or location. Please be aware that your medical provider will not likely be able to take your phone call immediately as they are serving patients. However, the receptionist at your provider’s clinic location will either take a message and share it with your DMG medical provider or have another qualified team member speak with you to answer your questions and/or address your concerns as quickly as possible.
How do I schedule an appointment with a DMG medical provider?
Whether you are a new or existing patient, please contact the DMG medical provider’s location to schedule an appointment. What health insurance plans are accepted?
DMG Children’s Rehabilitative Services (DMG CRS), Valleywise Health, and our other community partner locations accept most major insurance plans, including Medicare and AHCCCS. To find out if your health insurance plan is accepted, please contact the clinic location at which you want to receive care. How can I pay my DMG bill? Can I pay it online? What payment methods are accepted?
For your convenience, we offer a few options to pay your DMG bill. Having your DMG billing statement available when paying your bill will save you time.
- Online with a credit or debit card
- By phone with a credit or debit card: Call DMG’s billing department at (602) 470-5075 Monday-Friday, 8am-5pm AZ time.
- By mail to the address noted on your DMG billing statement (Lockbox 860183, Minneapolis, MN 55486-0183). Please do not send cash if paying by mail.
- In-person at DMG billing office, 2929 E. Thomas Rd., Phoenix, AZ 85016. Cash, credit or debit card, personal check or money orders are accepted.
I received a bill for medical services provided by a DMG medical provider, but I don’t know what it’s for and/or recognize it. How can I get more information? What should I do?
In addition to DMG operating DMG Children’s Rehabilitative Services, DMG medical providers also serve patients at Valleywise Health facilities, COPA Health, and other community partners throughout Arizona. The bill you received may be for services rendered by a DMG provider at one of these locations.
For more information, contact one of the numbers below based on the “Provider ID” on your bill. Please note that if you call the wrong number, they may not have your record on file and, therefore, will not be able to provide you the needed information.
- If the Provider ID on your bill is “MPRMPR6,” please call (833) 538-8540.
- If the Provider ID on your bill is “MPRMPR4,” please call (833) 538-8544.
- If the bill is from DMG and the Provider ID does not match one of the two noted above, contact DMG’s billing department at (602) 470-5075 Monday-Friday, 8am-5pm AZ time or email Billing@DMGAZ.org.
